The machine is skipping stitches and is probably out of time. Change the needle, rethread the machine and try it again. If it still skips stitches, try lowering the needle by about 1/16 inch and see if it works better. If the timing is off you should take it to a technician for a complete service.

Cut 6-8 inch piece of thread of the spool you are going to use for your project
Take the needle you are going to use for the project
& insert the thread thru the eye of the needle
& lift one end of the thread to a 45 degree angle
& if the needle is the right size for the thread it will slide down the thread
But if the needle hangs on the thread you need one size larger needle for the thread

Take out the thread. Now cut the end of thread using a pair scissors. Now try that again. Once i had the same problem and later found out that the uneven end of the thread caused problem by refusig to move around freely hence preventing the stich formation
