You have entered the pin code wrong 3 times now you are required to enter a puk code to reset the pin code.
imei has noting todo with puk code which is on the sim card which is not related to the phone at all.
The puk code is sim card spesific it comes with the sim card when you get it or you can get it by calling your phone service provider of the sim card to get the puk code.
if you have entered the puk code wrong too many times you will make the sim card useless and you will have to contact your phone service provider for a new sim card, it will probably cost.

You need
to call the phone company that released the SIM card, they can retrieve the puk
code from a database for free.
In your
case you need to contact your SIM card provider technical support to unlock the
SIM.
The PIN
and PUK are related to the SIM card, not to the phone.
If you
replace the SIM card with a different one, then the phone will not ask for the
PUK anymore.
