to do this start with:
1) Going to the settings app (the one with the wrench on it)
2) Click on other settings
3) Scroll all to the right until you hit the 4th page of options
4) Click Format System Memory
5) Then click Format
6) Wait for it to finish
7) ???

Make sure you have the specific Action Replay for 3DS and not the one for DSi / DS lite.

Try to reset setting for 3ds, it mean you need to have default setting to avoid this error. If it still occurs then format all the data from 3ds.
