My Onkyo HT-R380 Receiver is connected to a Verizon FiOS Motorola QIP7232 2 STB and a Vizio SV422XVT HDTV. Connections are as follows: HDMI from STB to AVR HDMI IN 2 HDMI from AVR HDMI OUT to HDTV HDMI IN 1 Optical Audio Cable from HDTV AUDIO OUT to AVR (for using Vizio Apps like Netflix & Pandora that do not require the STB) Everything works perfectly when watching/listening through the AVR speakers. Sound is also heard from the TV speakers, which I mute when listening through the AVR speakers. The trouble comes when trying to listen to audio through the TV speaker in HDMI THRU mode. Video is passed through to the TV, but no audio. Amber "HDMI THRU" indicator is illuminated on front of Onkyo AVR HDMI Settings on the AVR are as follows: HDMI Control - "ON" TV Control - "ON" Power Control - "ON" ARC - "Auto" (by default) I have tried changing TV Control setting to "OFF" which then allows selection of AUDIO TV OUT setting to "ON". Still only video is passed through to TV, no audio. Again, video AND audio are present through TV when AVR is on and audio stops when AVR is placed in HDMI THRU mode. Any ideas?

I am going to take a stab in the dark here...
Did you plug the hdmi from the receiver to hdmi via the monitor hdmi output? this is needed so when you change the device on the receiver ie. between setop box and dvd the monitor changes with it accordingly.
You may also need to go to the setop box to set it to use hdmi output at the right frequencies too..ie. 720p 1080i or 1080p.Also set it so it only sends the picture via hdmi but audio still with optical out.

Your symptom sounds as though you may have to turn off any surround sound settings that you have selected in the other modes.
Most amplifiers use the surround portion of the amplifier to power the B channel speakers.
Since you have sound when using the radio portion, I would try turning off the surround mode. For example try turning the amplifier to just stereo and see if that gets the B channel speakers to work
