A "Subsidy Unlock Code" is a code that is unique to your phone.
The code is assigned by the original service provider the phone was manufactured for so that it cannot be taken to another carrier for activation without their permission.

Most times you will receive the message " Enter Special Code " when you insert a SIM card from a diferent carrier. Motorola cannot provide subsidy unlock codes. You will need to contact the original service provider for further assistance.

Visit a Verizon store. Perhaps you have a bad SIM card or your phone's radio antenna is damaged.

If it's asking for the pass code it should eventually ask for your Gmail info and they have ways you can recover the login info if unknown. However once the pass code is set the only way to get past it other than what I've already mentioned is a factory reset.
