Where do i enter puk code on iphone 4s

For PUK code, you need not to service your phone, it's not the problem of your handset, it's from the service, What you need to do is, call the customer care of network operator (service provider) using another same service provider number. eg. if you have T-mobile, then borrow your friend's mobile who is using T-mobile network phone, there should not no charge, and tell the customer care your problem, he/she will provide you the Code after few questions with you.
