There is a utility that is built into windows its called DISKPART
Make sure you select the USB drive not your OS/Data drives

I have seen this happen a lot of times with memory cards and formatting is the only solution for this and in the formatting options choose FAT32 as it seems more reliable than FAT. I have last formatted(FAT32) my card two years back when I had problems like these and I haven't had this problem since then.

1.Insert card into your phone but don't access it through phone.
2.Run FExplorer and Open the path C:\system.
3.You will find a file called mmcstore ,rename the file mmcstore.txt
4.Copy that file(mmcstore.txt) to your pc and open that file in notepad.
5.You will find your password in that file.

Maybe your memory card is somehow corrupted? Unfortunately you would have to format your memory card and lose your data.You can however try using a program like recuva to try and recover the files.
