Cut off water supply to faucet remove allen screw that hold handle on remove handle unscrew bonnet nut then take out cartridge and put in new one.Make sure you note how old came out so you can put new one in correctly

There is a small set screw on the handle it may be covered with a small cover that needs to be removed to reveal the set screw.

I assume you mean the separate H/C valve body as opposed to single handle(bath/shower).

Should be a pretty easy fix, you need to remove the faceplate for the faucet then there should be a gap between the wall in the faucet valve and you should be able to see two screws that are holding it on to the wood stud in the wall, you can remove the screws and put a couple washes behind the valve that way it will bring it out further and you should be able to get your handle on it without a problem.

Pop the cap off the faucet handle and tighten the screw under it.

If this is a tub/ shower valve, you installed the cart. upside down. Reversing it should solve the problem. You might have put it in according to instructions but the hot and cold might have been reversed when the valve was plumbed in.

The bottom half of the handle spins CCW to losses the handle from the body. You can then tighten the screw inside the handle.

It takes new valve cartridges and sometime complete valve bodies. Call Kohler, it should be covered by their lifetime warranty. They are very good about service.
