It appears your crib may be on recall. Please check this website by the CPSC. CPSC GRACO CRIB RECALL LINK If it is recalled, some Walmarts will let you return them for a refund or credit, no receipt required. Take a copy of the recall page with you. Good luck and if I am wrong repost with the Date of Manufacture and I will look deeper.

Which conversion instructions are you looking for? I have them for the Graco 4605W and would be willing to scan and send them to you.
