Wont read disc
If you have one of the slim models that has the pop open lid, first thing to try would be cleaning the lens. Usually, I use either isp alcohol or windex and a cotton swab. Just dip the cotton swab in the cleaner and squeeze out the excess liquid. Then GENTLY rub it over the len and give it about 5 min to make sure it dries.
If it's one of the original fat ps2s you may want to try one of the commercial lens cleaning discs (though I haven't had much luck with disc based cleaners.)
If cleaning the lens doesn't resolve it, it's most likely the optical assembly (that houses the lens) has worn out or possibly even the spinning motor. In either case, it may be cheaper to purchase a used one than repair.

Game disk was removed - ERROR
There can be two possible reasons. The first most likely reason, if it is on only 1 game, is that the CD has been damaged too much and cannot read everything on the CD that is has to read on some times.
The second option is that your Playstation is the problem. This is only if it isn't only for one game, but for multiple. This would mean your disk drive is damaged (usually by having an older console).

Disc read error
take the ps2 apart and clean the laser eye with rubbing alcohol
takes about 10 mins
( i was on my third game before i found this fix) everything works great now! rock on!

All games in my computer are not working
If the a game will not start on a Playstation 2 its a defect disk.
trying cleaning it.
Remember to not trie and clean the disk in cirkular motions as that can lead to more dust gets in to the disk.

Longer wires for inside te playstaion?
I'm not sure what wires you mean, but Radio Shack, or similar store, should have anything you need, or at least the means to splice longer wire into the existing line.

Then, yes, they sell all kinds of them. It's called an IDE cable.

How to Install PS2 Modchip?
what you need: an IDE harddrive (maxtor or seagate works best. ive got a maxtor diamondback or whatever its called), the online adaptor and hdloader. hdloader can only be found on the internet so google it. if youre interested in my setup then you need the original hdloader then you can go to ps2hd.com and find all the other programs
if youre interested in doing cogswap (play burned games without a chip) then also search that site for the ps2 exploit disc and cogswap.
each harddrive is different and can require differnt jumper settings. look on your harddrive to figue out what is cs or check te company's website
where you put your harddrive on the ide cable matters. when i first installed a secondary harddrive in my computer i had to shift my original harddrive down one position to make room for the secondary and allow it to take the secondary position on the ide cable. if you switch them then your computer will tell you "cannot find primary harddrive" and will not boot.
one thing that i didnt mention is that you should also ground yourself after grounding the computer. your body can carry static electricity and if it gets discharged inside of your computer you can destroy components like RAM, the motherboard or even harddrives. to ground yourself touch a part of the metal casing of your computer.

PS2 memory card
Have you tried both Memory Card slots? Maybe Final Fantasy Tactices doesn't understand a 16 MB Memory Card since the official Sony ones are only 8 MB's. Maybe you can try one of those?

Guitar Hero III on PS2 Saving Failed
Could be. Maybe the game doesn't recognize the 16 MB. You can try an original 8 MB Sony one. They're not that expensive anymore. But first try the other Memory Card slot.

Parental control password
i don't know if you changed the parental control code or not but when it comes to put the code press select and then insert 7444 and it will delete the password and will ask you to put the new one then add any three digit number and then when the movie starts press select and go to setup and then parental control tab and turn it off now there will be no passwords..........

have you tryed other games?
If other games work, then your disk is probably damaged try a disk cleaner solution.

Kingdom hearts 2
Here's a link from GameFAQs. You can view some walkthroughs written by gamers who already completed the game. There probably is some advice about how to kill them all in 40 seconds. Also it might benefit your further gameplay if you get stuck somewhere.

Be careful though, those FAQs usually contain spoilers about the plot of the game as well, So my advice to you is: only look up what you need to know every time, do not read ahead.
