Go to Settings>Initial Setup and scroll down until you see Master reset (for settings only) and Master clear (for erasing everything in your phone and restoring it to its original state or setup). You can choose any of this.

For fixing this problem you need to replace the LCD of your Moto. Buy the new LCD and get an T6 screwdriver, as for Nokia screws. This is not very difficulty to replace.
