There's a button on the front of your 360 and the same button is
on the front of the controller. You hit the button on the 360 and the
green lights around the power button start to circle. Then you hit it
on your controller and the green lights around the xbox guide button
start to circle. In a second or two they'll find each other and both
will flash at the same time, and it's synced up. I've linked a video for both the Xbox 360 and Xbox 360 Slim.
This can be an over heating problem. Make sure you are allowing for adequate ventilation around your Xbox 360 unit. If you are sure the system is getting enough venation, you will need to power off the system and power it back up. If you continue to have problems you can contact Microsoft via their website at www.xbox.com

You can also register your system so if need be you can have the unit serviced under Microsoft's warranty.
Take out the screws on the back of the controller, open it up and clean out the area around the analog stick. The inside of the controller is actually quite simple.
The guide button won't work until its synced,you need to push the button on the console to look for controllers then the button on the controller and it will sync it and then it should work fine. If that doesn't work then try hitting the button on the controller first then the one on the console.
If you are using a wireless controller, the most likely issue is that the batteries in your controller have gone dead. To replace the batteries, simply do the following:
1. Open the battery case by pressing the release button on the top of the battery pack which is located on the back of the controller.
After that, if you are still having trouble you may just have a bad controller! I would recommend contacting Xbox support for further help. They may replace the controller free of charge or if you bought it from a store, you could return it due to it being faulty.

It means that the controller battery holder is loose and the batteries are loosing contacts with the terminals, this happen specially when the controller vibrates. Try to put a piece of paper or plastic in the inside covering the body of the battery or a tape to thicken it a little then place the battery in the battery pack holder and it should work fine. seems like it might have a need for recalibration.
turn the controller on, then press the radiofrequency button on your xbox (located in the fornt part, really small button) then press the same radiofrequency button on the controller (near the bumber buttons on the back, upside down)
