I reckon yer phone is stuffed as there is a loose connection in the headset jack and thus the phone thinks there is a plug in there all the time. The only suggestion l can make is to get nail 3.5mm the size of the plug and stick it down the hole an give it a wiggle (at yer own risk of course ) this should shake up the connections an should be all good again.

It sound like your power supply is not providing enough output for the radio. Are you by chance using a wall wart for power? You need a dedicated 12 volt power supply, made by Astron, QJE, or Pyramid.

Connect the headset to the charger as if you were charging it, but keep one side in your ear. Once connected to the charger, hold down the power button. It'll beep a few times and then stop. try to do a Hard reset on the unit and then reload your freq list from Chirp. It happened to my UV-5R-v2 Baofeng. It worked fine after that.

you may have a bad squelch pot on the radio. try turning it fully in one direction an back in the other direction. it also could be that the Pot is dirty get some contact cleaner and open up the radio and squirt a very little cleaner into the open spot of the Pot. then rotate the know several times. if you do not have something like CRC QD Contact Cleaner you can get it in a good hardwood store like ACE or TrueValue, and even Home Depot. you can use only 91-99% Electronic Grade Isopropyl Alcohol, do not use the 70% Isopropyl Alcohol.

The usual cause is that the transmit button on the mike is broken or stuck in the "talk" position. If you disconnect the mike and the radio, then turn the radio on and it still is showing transmit, then the issue is inside the radio or the connector for the mike.
If the radio stays in receive mode with the mike disconnected, look at the mike connector for any bent pins or damage to the connector. If you don't see anything, try spraying electronic contact cleaner , the could be some corrosion that is causing a short here. Another thing, if you have it, is to use a different mike and see if that works properly.

The overall reports on this radio, which appears to have been discontinued, is that it has a number of problems and needs to be serviced and the circuits aligned by someone with the appropriate skills and equipment. If you purchased it recently, from all reports, see if you can get it serviced under warranty.
otherwise, find a good CB repair shop and have them check it. The likely culprit is a bad transmit circuit. Not a easy fix unless you have the tools to test properly.

Most headsets do not have a reset since there is nothing to reset. If you need to pair it again with a phone, go into the settings of the phone, find the bluetooth device list and tell the phone to forget the headset. then do the pairing again, after fully charging the headset.

If the radio is designed to use a two prong connector, then you will need to replace it with the same type. Generally, one prong is for the mic and the other is the speaker. There is usually a common ground on one of them and possibly the push to talk on the mic prong.
You shouldn't have much trouble finding a two prong replacement mic, eBay has many for most radios.

Your antenna should not make any difference with power or receiving. Is the scanner mobile or in a room? Hard to imagine the connector being broken. You could test the antenna or even the BNC connector on the scanner. Try another antenna. If you wiggle it, does it start receiving? You may have a short in the BNC to the receiver. Other than that, look to power issues.

Radio frequencies are different from country to country. NZ will use different frequencies than Canada. Canada and the USA have agreements in place because they border each other on what freq. is used for what. NZ likely has its own list. Your radio is not programmable to NZ frequencies. Sorry!
