Try one or more of these options:replace the carriage PCB ,replace the head/carriage unit,replace the main PCB.

Are the inks full? Have you tried the head cleaning utility in the printer drivers? If those don't work, get the inks out of the printer, soak the print heads in thinners and try the nozzle cleaning utility again. If that doesn't work i think you may need some new print heads/ink

Paper jam, do a thorough inspection looking for any stuck bits of paper.
