You have to report it in your Telecommunication company or network provide to block the sim and the phone you are using,if someone took your phone,he/she continue using the phone and the bill is running too,also avoiding the phone accepting any sim if it's already block.the operator will ask your sim number and the IMEI of your phone,then they will immediately block the phone.

The problem is something to do with your headphone jack. It may be malfunctioning or you might have gotten something in it like water. I'd say take it back to the store, have them look at it and if it's under warranty have them fix it. If they tell you there's liquid inside of it.
