It is the Supplies Activation code. It can only come from Xerox themselves. It ties you to buying supplies from them. If you put in a refilled toner it locks the machine.

Press and hold the 0 key for 5 seconds and then while still holding, press the Start button. Enter password 6789 then press confirm. Select Adjustments/Others, then initialize HFSI Counter then enter code 954-804, then select reset current value.

This will sound a silly question but have you tried changing the settings on the control panel to Black and White? The button should be on the lower left side of the main control panel. I had that problem and it was that the paper was damp. It didn't feel damp, but keeping the paper in a hot airing cupboard fixed the issue. If that doesn't fix it, it may be a timing issue that the fusing unit isn't coming on early enough. There must be a switch.
