PUK code can be given only by your service provider which is specific for each and every mobile number.
Contact your sim customer care department over phone. Its free of cost to get the pin or puk no. If you cant contact through your phone get some other phone and contact them.
Go to the nearest service outlet of your service provider they can also help you.

PUK code is called as PERSONNAL UNBLOCKING KEY.
Your phone will ask for PUK code when you entered the PIN code wrongly several times depending upon the mobile make. The mobile asks for PUK code so that unauthorized persons do not use your mobile for wrong cause.
To get PUK code please contact your service provider through phone or go directly to their office. They will provide you the PUK code for your mobile. The PUK code is unique for each mobile phone model.so that you can open your phone. When you planning to call your service provider you cannot use your mobile as it is fully locked. Please use some other mobile with same network to call your service provider.
This PUK code prevents thieves from being able to make use of stolen handsets, that is why it can only be given by the service provider only.

While you get the PUK code ,the service provider customer care officer will ask for your personal details to check whether the mobile belongs to you only. If they are satisfied with your answers they will provide with the PUK code
DONOT enter the PUK code several times wrongly,It will block your sim card permanently.

To get your PUK code you need to contact your network providor. Tell them your phone number and they will calculate your PUK code. If you do not know the phone number of the phone, you can not get PUK code.

Ring your service provider to get PUC no.

This problem is very common and most users think that it's related to phone, but in fact it's not, this is only with your sim card and is for security reason. We can't use
software or hardware or other tricks or methods to unlock it, the only solution is from the Service Operator/Network Operator.
If you keep entering random codes/unknown numbers several times, your sim card will be rejected and will not be usable.

Your only solution is to call the service provider (Network operator's customer care), as them to give you your puk code, they'll promptly give it to you after few inquiries to make sure you are the true owner of the sim card.

There's no other alternatives, or not other options that you can get the code, so don't play around, rather make a call to the customer care of the sim card network operator and get the code.

I know for a fact that you can find your PUK if you are using AT&T by logging in at their website. For most service providers however, you can just call them, and they can look up your PUK.

please sir i wish to retrieve my Sim back i lost the code and my Sim was blocked how can i open it or what am i going to do

First, I'd try running a CD lens cleaning disk, see if a dirty lens is the problem.

Second, you could try the TEAC website, see if there's a firmware upgrade for you to download.

Third, you could take it out of your PC, see how far you can dismantle it and give it a clean.

Finally, which you may have to do after option 3 is, yes go buy a new one.

if PUK code what entered to many times then you can't do nothing else with it..

Contact customer service of operator where you have card from, they will have to make you new SIM card and disable current one. PUK was last step in security, now card is dead.
