There is nothing online saying how to repair this, so I'll guess. The little washer will fit anyone of those metal spindles. It allows something to spin without binding in the black housing.My guess is the tall one or the one to the left. The knot on the end of the string should fit into a slot on that tall spindle and the string should sit between those indentations. The should be tension on the tall roller, so you will need to pre-tension it. You can pull the plastic string guide grommet up to do it. Then push the grommet back in. Put the housing back on top.

Did you check the metal pin and spring terminals the batteries sit between? If a battery powered toy is stored with the batteries still in the terminals can corrode and block the electrical current from the batteries. Try cleaning the terminals with a cotton bud and Very Fine sand paper. A cardboard nail file works well for this.

To gain access to safe without code proceed with the following simple instructions:

Put the crank handle in the down position and then use the arrows on the gears to guide you. The two gears that hold the two spirals should be closest to you. Make the two arrows on the gears point directly at each other(keeping the crank in the down position). The third gear's arrow (furthest from you) should point directly at you bisecting an imaginary line formed by the other two arrows pointing at each other. Playing with Lucky is pretty simple but does require some learning. Lucky is programmed to respond to 15 different commands but the commands must be issued the same way each time. First, you say Lucky's name to engage him. Once you say "Lucky" (or whatever name you give him) he will then respond to you with 2 barks. After he goes ruff-ruff, then you can tell him what to do. The commands are as follows: 

Speak to Me - he will sit or stand & bark 
Come Here - he will run forward and back 
Pounce - he will crouch and pounce excitedly 
Lay down - he will lay down on his tummy 
Sit - he sits 
Want a Treat? - he stands and barks (begs) 
On your head - He does a headstand 
Go to sleep - lays down, snores and enters sleep mode 
Sing a song - he'll bark one of 2 songs 
Good Dog - acts excited by shuffling his feet 
Bad Dog - he crouches down

Playing with Lucky is pretty simple, you command and Lucky performs. Well, Lucky performs most of the time. You need to be sure that he has your attention (barking twice) first before issuing the command. A few of the tricks are a bit more complicated - Put 'em up & Tug of War but the rest are very straight forward. I do notice that both my husband and I had the tendency to want to 'yell' or raise our voices when commanding Lucky - but that isn't necessary.
