volume - personal taste - usually just high enough that you notice the bass - try turning down until you don't notice it then turn up one notch in the switch

phase - depends on distance from other speakers - try initially setting to 0 degrees

cross-over frequency - depends on your other speakers - try setting at 60-100Hz

Let's establish that your feeding it a good signal first. How is it hooked up and to what?

A quick kand dirty test of it would be to jack in a CD player to its RCA input. It will tell you if it's alive and functioning.

THEN we'll troubleshoot the signal source end.

Normaly C26 has turned bad. It's 22uF/25V and is easy to access when module is disassembled.

disconnect offending unit. then play see if same response occurs on the alleged normal unit using the offending cable interchange cables see if you get same result. Means it's bench repair ,internal electronics require specialist equipment to locate the offending item.
