Where is the tension dial

Stitches loose and do not want to stick
You have not provided enough of a problem statement to get a good answer. (1) Has this machine ever worked well for you? (2) Are you sure you are using the right kind of thread (sometimes the shape of the thread base makes a difference (3) Have you experimented with different settings for thread tension using junk fabric? (4) Are you using the same thread in the bobbin as well as the top thread?

How does the thread on top of machine stay put
The steel piece on top should extend upward. You probably have 2, one for sewing and one for bobbin winding. It may be that the steel piece is not fully extended upward or has broken.

How does the thread holder work
It sounds like you are asking how to use the spool cap. These are put on after the spool of thread to hold the spool in place.

How to make a butten hol
This is a 4 step process. You start at the first step and it sews backwards first, Next you perform step 2 to sew the bar tack. Third you sew the right side of the button hole as it comes towards you. The final step is another bar tack which is the same as the second step.
