I get
i had the same problem as you did.. i found out that all i had to do is blow the dust out of my head phone socket and the problem was resolved. i now this sounds crazy but it worked.
My Denon AVR 589 does
Is there a switch on the back of the receiver that will switch the sub signal from digital to analog? And the same switch might be on the subwoofer itself.

Keeps clicking between speakers on and hp input
had same problem with my denon 1910 .... clicking with no sound and some modes indicating not available. Blowing the dust out of the headphone jack solved the problem !!!!!!!!!!!!! ... the denon people could not even figure it out. Thanks sooooo much.

Assuming that you mean Dolby digital 5.1, confirm that you have a center channel properly connected and perform the following;
Using the remote, enter the menu on the receiver and navigate to the selection labeled "LEVEL." Toggle through the selections until you reach the "CENTER CHANNEL". You will be able to use the remote to increase the volume of that channel in .5dB increments.
Does not go in surrond
Some models can detect the speakers attached. Do you have all 5 speakers connected as required for surround sound?
