Toilet not flushing completely
Prelude:
My toilet had the same problem a few times. I managed to clean it out because the pipe is somehow connected to the downstairs drainage pipe in my house. What fixed the toilet clog was going to the source of the problem and pouring extremely powerful hair clog remover chemicals in it (I did have to open all the windows and get the pets as far away as possible and wear a mask). After the specified amount of time waiting, I went to go flush and it worked.
**Answer**:
I would recommend doing the same thing but only if everyone in the house is gone for 1 hour. Also I would air it out by leaving windows open, not first floor doors or windows though. It can be deadly if inhaled for extended periods of time. The more powerful the cleaner, the better the results but the deadlier it can be if inhaled.
Try this.
My second guess is that it may be the plug inside the toilet's tank. You should check to see if it is working properly. Try flushing with the lid off and watch the plug to see if it is working properly.

Plastic fitting at main hose tie in leaks
Most hardware stores carry a good variety of fittings. (Plastic, Brass and iron). Just be sure the leak is the fitting and not just the hose connections. Don't forget to turn the water supply off before cutting the lines.

I have 5 kohler pressure
There are two holes in the top of the bowl where the tank attaches. As you indicate, one hole delivers water to the jet at the inlet to trap passageway, and another smaller hole to direct water to the rim.
I have seen in some cases the gasket that seals the tank to the bowl block the smaller rim-fed hole. All you need to do is knotch the gasket where the hole is located and re-install the gasket so the knotch is located over the hole.
