I have a Life Fitness T3-0 which shows a
FET Stuck on Error / Err 5 - This error is probably caused by the driver transistor on the motor controller is detected to be shorted out. The fix is replacing the motor controller.

The common cause of the startup error is the Opto Sensor (Speed Sensor),either there is a poor connection of the opto (speed) sensor or it is out of adjustment or bad.
Check the opto (speed) sensor connection and wires. Make sure the opto (speed) sensor is evenly spaced over its disk. Replace if you find any broken or damaged wires. Inspect cable and connection of opto (speed) sensor. Repair and replace damaged cable or connection of the opto(speed) sensor.
Inspect alignment of opto (speed) sensor. Sensor should be centered evenly over target. Adjust or replace opto (speed) sensor if necessary
Your sensor might look ok but you have to inspect it very carefully.
The belt slides to one side (the same side) always
you need to tighten the w/belt. the side that the belt goes to tighten 3 quarter turns. now on the other side loosen 2 quarter turns. turn on the the belt on 3or 4 setting the belt should start moving over when it get to where you want it tighten the sid e you first loosen 2 quarter turn. then run it at higher speed to see if it stays its trial and error remember rightie tightie leftty loosie. in the end you want equal tension with the rollers bolts ps good tension is 2 -3 inches up when lifting up the sides of the w/belt
I have a T5 treadmill (same as the T3,
somthing is tripping the fuse.take hood off and check all connections with power off maybe somthing is loose or touching to trip the fuse and the red led is normal. also always plug your tread in to a surge protector. most likely the controller board

calibrate the belt with a Allen wrench 5/16 inch turn in the direction
of clockwise at a speed of 1.5 to reach 4.5MPH you must turn the key
gently to prove that you slide about 10 seconds walk.Use WD40 oil on the joints to make it noise free.

1 reason could be belt and deck(motor draws a lot of amps and belt and deck heats the motor up)
just try wax or libricate compatible lube and try (belt and deck lube you can buy from gympart.com
