the 2 fans should both work you have a fan for the a/c and a fan for the water coolant, when a/c is on the fan should be going, depending on a/c settings there could be 2 fans for the a/c as the a/c fan is next to the radiator you can get heat crossover if the a/c is not cooling down. if engine is fine without the a/c on then i would look at the fans.
This should only take about a hour to change plus the sensor cost I think this car has 2 o2 sensors but i would recommend that you a OE type sensor (bosch) they are not cheaply made But here in DIXON I would do this for $198.00 for sensor and $60.00 labor total $275.00
If sensor is stubborn and removal hard I suggest to spray sensor with a penetrating oil for couple times this will soak into the threads to be removed easier
You need to drain the tank and fill it back up with Deisel. Also, if you ran the car make sure and change the fuel filter. It may start a little hard and sputter at first but it should be okay. These accidents happen once in a while but it shouldnt hurt as long as you dont run it with the petrol in it.
the smoke is from the oil leak..oil blowing onto the exhaust while driving....where is the oil leak? at the rear of engine? check to see if your valve cover gasket is leaking...it could also be a rear main seal in the back of engine, maybe even the oil pan gasket.

You have a leak, you just need to find it.
Look for leaks in the hoses. Do a pressure test.
The most common reasons a Volvo S40 has a coolant leak are loose hose connections, a broken radiator, or a failed water pump.
They make a cd player cleaner you can buy about any audio store , I would give that a shot first. Looks like a cd with small brushes on it. The laser is probably dirty.

This is a strange code that doesn't appear to have one definitive reason, it's a bit of "you have a bit of a problem, but we aren't sure". Just Answer dot com mechanic says it is the MAF Sensor.
