Not under the hood. The "mode" door, actuator and vacuum lines are on the heater box under your dash.

Rear Floor Vent heat only

Sounds like your heater coolant hose door flap is broken or loose. May want to check behind the heater controls, if your components are mechanically operated. If electronically, may need to perform more troubleshooting before replacing this component.

THE DEFROST BLOWS OUT HOT AIR, BUT THE FRONT SIDE WINDOWS AND THE TOP OF THE WINDSHIELD FOGS OVER. HARDLY ANY AIR COMMING OUT OF THE VENTS ON THE SIDE OF THE DASH THAT BLOWS ON THE FRONT DRIVERS SIDE AND...

You didn't specify what type of vehicle.
If you have very poor air flow out the vents, later model vehicles have a hepa filter or cabin filter in line form the blower motor to the vents. It can be plugged quite easily and drastically cuts down the air blown into the vehicle. Check to see if they are plugged. Secondly, Check your coolant level in the rad and reservoir. If the windows fog up when cold and air flow is good it could indicate a leaking heater core. Check the passenger side floor for signs of coolant again I don't know the type of vehicle, some heater cores are located at the firewall under the hood.

Heat won't come out of the vents at your feet in '98 Chev.Siverad

Look up in the vents with a flashlight, some times an errent object placed on the dash finds its way down the defroster vent and into the plenum. These items tend to keep the doors that dirrect the air from moving.

Over heating

it is most likely your heater core possibly the hose connection but really its the heater core

Heat

So I am guessing the heater core, the only other thing it would be would be any leak that is allowing moisture in the car. is the floor board wet on the passengers side?
