PO122--ETC Throttle Position (TP1) sensor Low Input
PO223--Throttle Position (TP2) High Input
PO354---Ignition Coil D --On an LS, that is not
a used code,another engine it would be cylinder #4
What I believe you have,having diagnostic experience
and also owning a 2004 LS,is a Bad COP Ignition Coil
on cylinder #4 that is bad enough to spike the PCM
& come back on the TPS Sensor
You ignore the codes and buy 1 new coil for $59.00
and put it in #4
If that doesn't help,remove the new coil and walk it to
another cylinder and drive a couple days,continuing
to move the new coil.
If you had some very expensive diagnostic equip
you could go with a different approach
The last thing you EVER want to do, is go to
a dealer,you may leave there the same as you were,
and a bill for a grand per visit.
IF you have a V6 and I hope you do not,
then if you get involved with cyl #1 thru 3 where
you have to remove the upper intake,do the spark
plugs on that side and all new coils while in there.
Save those coils for your future diagnostics
These coils fail at a rate of 1 or 2 a year on all fords
Is there an after market coil that can be used?
On a 3.9 V8 the coil is Wells (brand) 5C1179 $87.54/$48.64
at the auto parts store
I called Wells last week & the C1234 is the same coil, but for
Auto Zone ( Around $46.00)
If you have a V6 you will have to look them up on the Wells site
or other brands.

My personal mechanic advised I goto the dealer and yes it seems as if they are doing exactly what you said. $500 for 6 broken coil connectors. That was this morning. Right back to dealer now with check engine light on again. I originally took the car to Autozone and their read was a modulator. Any advice from here?
Stay away from the dealer. Why would you have anything broken?
Someone over the years did the damage to your car. Try another repair
shop. Why can't they figure out anything on your car & resolve.
They can find the bad coil in Generic Mode $06 & have to use an oscilloscope
& take it with them on a road test. You can't find a bad coil at the shop.
The fuel tank has to be over 15% or the misfire monitor won't run
