One or more of your pulleys may be damaged or misaligned. Remove the belt and inspect the pulleys for cracks and/or sharp edges. You can use a stiff straight edge to check alignment. Also check that there is no play or wobble in any of the pulleys and that your belt gemstones is not worn or broken.
Something is making this happen - I'd say to inspect all the attachments connected to the belt - alternator, water pumps etc for excessive wear and/ or play.

Crankshaft Damper or pulley, could be the cause
It will deteriate over time and the rubber rot
You won't see the pulley move out of alignment
but it will move 1/8''
