Problem within the gear box or a possible misalignment of the gear stick with the lever movements.
Make sure there is no PLAY on the gear shaft . If so the gear must shift over or else the problem can be the selector jamming within the box.
If there is play in the gear movement then the bushes could be worn out- sometimes nylon bushes gets damaged.

Are you saying you can not move shifter while driving or it will not shift out of 3rd gear? If just the fact it will not from 3rd gear sounds like it could be in safe mode. Is check engine light on?
The gear may be stripped, can't catch other gear,have you been grinding this gear,will have to go into transmission and replace third gear,will have to wind second gear out and shift to 4th gear.May be better and cheaper to get a remanufactured transmission cause all parts are new versus just 3rd gear being replaced.
The 3rd Gear Synchro is worn or damaged, which is why it grinds when you try to upshift or downshift into 3rd gear. When it goes out completely you will have a very hard time shifting into 3rd gear, since the Synchro lines up the gears to make shifts smooth and easy.
Your transmission has 3 shift rails... 1st - 2nd 3rd -4th 5th- Reverse
If your 3rd gear doesn't work , you have an internal transmission problem, such as a syncronizer, or similar fail... There is not much you can fix, without removeing the transmission.

The car has just clocked 185,000 miles
Its a great car but a pai at the moment, I've had a service including fuel fillter change added fuel cleaners to the deilsel all to no avail
