You need to check your Idle Air Control (IAC) that is located on the throttle. When you change your transmission to a forward gear that gives the computer a signal and then the computer gives the IAC the signal in order for it to control the idle as needed, but if the IAC is damaged it wont work properly. I have had this problem in several cars in my shop and the cost of a rebuild IAC with the throttle is about $300, so good luck.

Dirty air filter, dirty throttle body...try 2 bottles of gas line fuel injector cleaner...has your car been parked for awhile? Heet gas additive will work on any low gas tank sludge that you may have. Hope this helps.

Idle rough in gear

Definitely the plugs too. If the wires are many years old, good to change them. If they are the original wires, definitely change them.
Also new gas filter, and new air filter.

Rough idle, rough in drive

Is the check engine light on ?
Have you checked the computer for trouble codes ?
You could have one bad spark plug even with only 30k on it.
Problem could also be a dirty injector.

Rough idle 95 astro van, I did a full tune up , plugs, wires, cap, rotor, air filter, and fuel filter and cleaned egr vavle.

I actually had to replace my EGR valve and the gasket. I clean the valve well and reused both. I don't think the gasket sealed well enough though. So I went ahead and got a new valve, gasket and changed them. It runs a lot smoother now.
