Apply a force of 100 N (10 kg, 22 lbs) and measure the deflection between the alternator and crankshaft pulley. Deflection: 10 - 12 mm (0.39 - 0.47 in)
NOTE:
On a brand-new belt, the deflection should be 4.5 - 7 mm (0.18 - 0.28 in) when first measured.
If there are cracks or any damage evident on the belt, replace it with a new one.
Loosen the through bolt and adjustment locknut.
Turn the adjusting bolt to obtain the proper belt tension, then retighten the nut and mounting bolt.

Your 1991 model Accord has automatic front retracting shoulder belts and, more than likely, the power retractor motor has gone bad on the passenger side front. If the shoulder seat belt is not in the retracted position, up close against the seat, when the car is running, the seat belt light will stay lit on your dash. Have the seat belt motor inspected and replaced if necessary.
If you have had the timing belt changed they might not have replace the balancer belt and it may have snapped. Or, the balancer belt might not be set correctly.
With the engine running are you getting any voltage off the alternator? check with a multimeter. Check voltage at battery while running should be 13 volts or more.
check fuses or see if something might have got jamed in the track and also try movin the part where the seat belt moves back and forth to see if the motor is dying
Is there vibration in steering wheel? Check for bent wheel then check axle...again. Remans are not always good and new ones are just a bit more expensive. Also, check the output shaft bearing for excess play. Also try rotating tires to confirm it's not there.
The vehicle speed sensor is failing. It is a small component bolted to the top of the transmission with two 6mm bolts. Easy to replace once you find it.
