After my truck (2003 S10, 4.3) sits after running it for a bit, start it and it will start missing and the check engine light comes on. The miss will go away after about 5 minutes running and the light goes out. I found a small puddle of anti freeze under the truck and looks like a small leak in front of the intake manifold. What can it be? I have 2 ideas of what it could be. A head gasket or manifold gasket.

Intake manifold gasket. when it leaks water will enter a cylinder causing the sprark plug to short out. when the engine gets hot enough the water will cook off in the cylinder allowing spark to take place. 80% chance intake manifold gasket, buy the upgraded part. 20% chance head gasket. Take the intake off and check for broken intake manifold gasket then buy parts.

What cause the engine to misfire when first start up after it been sit for a while?
Could just be a loose spark plug wire. Depending on how long it's been sitting animals tend to hide under the hood may have knocked it loose. Drive it to orileys or autozone and they can scan your computer if you have a check engine light on which in my experience with a misfire it sets off the check engine light or your gas could be old also.

check your spark plug wires. it sounds like cracked spark plug wire that is arcing. the best way to check this is at night , start engine and look for sparks around the plug wires.

If it only misses when the light is on or right after it comes on it is probably a bad oxygen sensor.

1999 windstar 3.8 starts up miss then quits runs good after that
Does the miss quit or does the engine quit. If it misses for a while and then quits missing, you've probably got a fouled spark plug caused by oil fouling the plug after the engine sits a bit. The root problem is leaky valve seals. The quick fix is try new plugs.

Check to see if your overflow tube againt the fire wall on passenger side is not leaking onto cylinder 4. Pull spark plug boot and make sure it is dry. When the heater core starts going bad it will leak out over flow right there
