Typically a belt will come off if all the pulleys are not perfectly aligned.
If you recently replaced a power steering pump and the pulley was not properly seated all the way down where the pulley front is flush with the metal shaft from the pump then you will shred the belts and they will come off. If you ONLY swapped the tensioner and idler pulley you may have installed the tensioner so it isn't parallel to the other pulleys but that would be very rare because it would have been difficult to get the belt on if it was not properly installed. There also could be a damaged plastic pulley that could be "cutting" into the belt which would also cause the belt to slip off.

i wold pay particular attention to the age and springiness of the tensioner. if the tensioner spring, not just the pulley, gets old it will not keep adequate tension on the belt, it will get too much slop in it and jump ship.

How to change serpintine belt 2002 3.5 aurora

Use a 3/8" breaker bar to move the belt tensioner. While holding pressure on the tensioner, remove the serpentine belt from any of the pulleys. Gently release pressure from the tensioner. Remove the old belt and replace with the new one. Reapply pressure to the tensioner and pop the belt over the last pulley.

Steps to replacing the serpintine belt

release tension on belt by:
-put a socket/breaker bar or ratched on the tensioner pulley bolt and use it to relieve tension on the belt
-slide belt off tensioner pulley. replace with new belt in same pattern across all pulleys and again relieve tension on the tensioner puller to slide the new belt back on
--you may need to remove the bolt on the tensioner closest to the pulley (i don't know just judging by the picture i found)
---if you don't know what the tensioner looks like look up the part on autozone's website for a picture you can match on your car

Just to make sure you have the right part (belt Tensioner), If you push and pull on the serpentine belt, look for the pulley that moves with the belt. That will be the Belt tensioner pulley, there is going to be a slot on the tension arm for a ratchet so you can take the tension off the old belt and slid it off the idler pulley or any one of the other pully's. Cant find drive belt tensioner bolt to loosen serpintine belt

The serpentine belt tensioner / pulley is on the front of the engine, and part of the belt 'routing'.

Its job is to put tension on the belt as it drives the other devices like the power steering pump, water pump, air conditioning compressor.

It is an aluminum casting that mounts with a single bolt to the engine. The pulley is attached to the arm.

There are a few ways to move / pry the arm inwards, thus releasing the tension on the belt for removing/installing: grab on the pulley bolt (15mm?) with a wrench, or some later models have an opening for a 3/8th socket wrench to attach to directly without a socket.

dodge and ply. all had this problem tensioner will tilt out under load and belt comes off happens more so when raining they make a kit to repair updated tensioner with a lip on pulley and grooves and a belt with ribs on both sides to keep it in line it is expensive i have replaced belt and tensioner but weld a ring around pulley as a guide to keep it from coming off or find a smooth pulley thats the same size with a lip on it

If the belt keeps slipping off the you need to replace the tensioner(its the pulley you have to move to get the belt on and off,and that part should be inexpensive to replace from any parts store.
