Check the blower speed control resistor pack, it is located in the air stream near the blower motor, that is most likely the cause of the inop blower, but be warned if the blower is drawing to much current due to wear the resistor will fail again, I believe the connector has four wires, 2 are heavy Gage

It is not the resistor that fails but the little thermal limiter next to the coils of wire that form the resistive element, it is round, looks like a little barrel.

Yea leafs and other debris can get in and cause the noise. Remove the blower, thoroughly clean the fins and reinstall. This should take care of the noise.

The sad news is that the new motor will probably whine also. The best way to fix the whine is to use the switch to select different speeds. It is is whining at one speed, it usually will not whine at a different speed, at least for a while, or until you turn a corner.
Speed issues are usually related to the switches resistor pack which is easy to replace.
