Needs a forward/rearward motor. it has two separate motors under the seat. very common and i replace them all the time. good luck
I had the same problem and it was a bad switch. Replacement switches are hard to find and expensive (~$100), so I bought a DPDT 2 position momentary switch to replace it. It works great, just be careful with the sharp metal under the seat...
I have the same problem. I'm female so I have no idea what a DPDT switch is and where to get one. Is this something even I can do?

Sorry, I should have stated that this is a cheap fix (~$15) but it does takes some experience with this type of project. If you are not comfortable working with 12V DC or cutting up the plastic trim at the base of your seat, this fix may not be for you. It cost about $10. First, check the voltage at the motor to see if the problem is in the switch or the motor. If there is no voltage at the motor when you press the switch, then the switch is the problem. Next, I cut a hole in the plastic near the existing seat switch for the new switch. Then you need to find the wires that supply power and the wires that go to the motor that stopped working. Splice into the power wires with a couple of splice connectors like these from the auto parts store http://www.amazon.com/Hopkins-49005-Scot... They are about $4-5 for a box of 6. Run 2 wires from there to the two center posts of the new switch. Run 2 wires from the motor to the switch (run to one side of the switch with a crossover to the other side of the switch).
