Did you scan for codes? If there are codes pull them out
provide me with them and I'll try to diagnosis this. This could be anything. The shifter may be defective the transmission range sensor aka manual shift valve may be defective. When your in first is it slipping?
Okay, two possibilities...1. The govenor valve is stuck preventing upshifts or 2. the foward clutch pack is burnt out. The first thing to do is check the oil. Check the level, colour and smell. If the level is correct but the oil is darkish a smelly then cutch burn out is the problem and the transmission will have to be rebuilt. If all this is ok, then the governor valve on the output shaft is stuck. Usually this vavle sticks when obstructed by contamination from a failed clutch pack or brake band.
Sounds to me if the fluid level is correct you have internal trans problem...defective clutch.....blown clutch seal.

How many miles? Have you maintained, serviced trans in past or just waited for this day to wonder what happened?

Your valve body and possibly input shaft nut need to be replaced. The valve body's booster valve is worn down and won't create pressure. Before it went completely out it slammed on the input shaft nut and loosened it.
You need to replace the valve body and retorque the input shaft nut. The transmission is not junk. The above needs to be replaced and change the filter and fluid and do it every 20K miles.

You need to check the transmission fluid level, if it is low the tranny will not shift. If, however, the fluid is to the proper level then you will need a new transmission.
Try putting new fluid in and look for a leak in the pan gasket, then all you may need is a transmission service. When the service is done the mechanic will look for pieces of metal in the old fluid, if found this is very bad and you will need a new tranny.
You might consider a new or used car, you can definately find a good deal out there!

Sorry but if your reverse is having trouble you will probably have to replace or rebuild tranny like me. Otherwise try new filter and fluid first.

Sounds like one of the speed sensors are bad. It has an input speed sensor and an output shaft speed sensor. You need to have the TCM scanned for codes to see if one of them is bad.
