Sounds like you need a new heater core. That is the side the heater core is on and when it starts to leak the water runs down on to the mat. you can take the old one out from the out side there is a door you can pop the rivets off and replace with screws it will be under a layer of undercoat so it might be hard to see but the heater hose is connected in the middle take it along when you get a new one to be sure it matches. It would also be a good time to replace the heater hoses too as they will rot and you will have to cut off the ends of the hose to get a good fit

or seal windshield... if you have not had a recent windshield replacement then i suspect the heater core

Based on our research the RAV4s with wet floors are usually, but not always, clogged sunroof drains or drain outlets. Other RAV4 owners found that the body trim surrounding their windshield or even the fender panel had degraded and failed, allowing water to seep in during heavy rain.

does you vehicle have the dual climate control on each side?? where you can choose how hot your air is, and the passenger can set theres?? when the floor gets wet thats usually a positive sign that the heater core is bad.. im not exactly sure how your lexus is set up, but id say either you have a heater core bad, or the blend door for the passengers side is stuck.. thats only possible though if there is indeed dual climate controls.. hope this kinda helps

Yes is it a sweet smell, is there wetness on passenger side floor does window get foggy. then yes heater core.

Wet rear floor

Have you by any chance jacked up the passenger side, or got vehicle on a hoist to make sure there isn't any plugs on the underside that may have fell out? They could be round or oval metal or rubber plugs, probably in that area.
