Check Transmission Fluid level. If that doesn't work, you need the dealership, or a transmission specialist. A high grade scan tool computer can diagnose with engine running, but can also record what is happening whilst the car is being driven. It could be the TCM or it could be shift solenoids.

What it maybe though is the "Bands" sounds like they are slipping, they simply may just need tightening, this is NOT a task that one can DIY. if it is this it isn't too expensive a fix either.
You will/should take the car into a place that specialises in the Lexus model. then ask ONLY for a "Quote' as to costings and what needs to be done, get appraised of it all, and go from there.
My Lexus es 300 most often when engine is warm will not shift in to last gear. Stop engine start it will shift ok when engine is cool. engine pings kock senores. I have a 94 lexus es 300 and I have the same problem the check engine and o/d light is on and it drives slow in first or second and then it will shift into 3rd with no problem i went to a tranny shop and they replaced the p0753 Shift Solenoid A Electrical and the p0773 Shift Solenoid E Electrical and drove around the block and after 15 mins the lights came back on and car acted the same way and got the same codes so he didnt charge me anything and was like he didnt know what it was so now im stuck with a car that doesnt have 1st or 2nd gear so if anyone can help im open for anything
If the transmission "brain" in modern cars senses problems it can keep the tranny from shifting. Usually leaves you in second gear as a safety and warning indication. I don't now if that year lexus had this ability but getting it checked on a diagnostic computer that read transmission codes should shed some light on your situation.
