Sometimes bad gas can make your O2 sensor give a false reading.Try a tank of super unleaded from a reputable gas station and some Berryman Chemtool fuel injection cleaner from Autozone

I think we all just live in a world of automobiles,that have too much high tech technology,when not so much is needed, just to keep exhaust emissions at an acceptable level

You gave the dealer techs several chances at resolving the issues, all be it, expensive, to say the least.
I don't see anything where you went wrong, or they let you down, assuming your good to go & no more issues
I assume that the head gasket was something that only moved & leaked internally under certain conditions & I don't know why or when the ECU came up as a problem,they don't fail often
No, bad brakes would not make the check engine light come on, you best bet is to wait for the check engine light to come back on and got to AUTO ZONEor ADVANCE AUTOPARTS and have then do a free obd2 check on it. they will bring out a small computer plug it in to the car and read what the problem is from the cars computer, then from there you will know what action to take.
With one injector replaced you may have a bad gas or somthing that is killing the flow to it witch could make the car act like that... need more help just let me know!
Have you tried cycling key for fault codes? Turn ignition to on (not
start) then off, then on, then off, then on, leave it on, and count
flashes of check engine light. flash,flash,pause, flash flash, is
code 22. 3 flashes, pause, then 5 flashes, would be code 35, etc. See
what you get, & get back to us with them if you need more help, and please include what engine you have in the van.
Smoke coming out from the middle of the two rear tires right? Sound like your rear differential is out of lubricant. Leftover lubricant is probably getting burned by heat from the friction. Have your rear differential lubed up.
