The p1131 indicates lean, there COULD be a vacuum leak? but there are so many causes of lean running that you could spend a week checking all of them. the p1479 indicates a problem with the cooling fan high speed circuit ....there is a CCRM (constan control relay module) up by the radiator, these have a bunch of relays in them and COULD be a problem, but you will need a service manual, electrical diagrams, and a scanner to properly diagnose. I would have a shop check it out. you dont want to spend money on guesses. stay away from any chain store operations, find a qualified shop that is able to diagnose this for you.

The clutch plates and their retainer springs have weakened. The clutch plates need replacement along with the retainer spring which keeps the clutch firmly gripping the cam shaft rotating flywheel, thus loosing the least amount of energy transmitted to the axle which in turn rotates the wheels. I don't what the error code means since you have not specified the make/model of the the sensor package, but the symptoms are exactly as you have described.
