Not too bad. Not much room, just enough to drop the starter. It is probably easier to go up through exhaust if you can get your exhaust shield off the manifold .

    Anti-Deterrent ECU:
This is also possibility especially someone else drive your car. This is very common if you lend your car to someone. The symptom of this condition is "No noise or sound at all" when ignition key is turned. If this is faulty, the symptom will be the complete ignition kill. Therefore, you will not heard and clicking sound or engine turning at all. If this is the problem, you can easily reset the Anti-Deterrent by using your key. Stand out side of the car and close all doors. Insert the key into the driver side door and turn the key to clockwise and counterclockwise for three four cycles. Basically, you are lock, unlock for 4 times and this procedure will reset the anti-deterrent ECU. If this happen to you, its quite scary at first, but the solution is very simple.

Where is the starter soenoid located on a 1991 Lexus LS400?

If the solenoid is clicking when you turn the key its very unlikely to be a relay fault, the relay controls the feed to the exciter wire which must be working for the solenoid to be clicking. If you haven't ruled out the battery completely do so by attaching jump leads, if that makes no difference you ideally need to check for a volt drop between engine and battery earth terminal and also main starter feed to battery positive, (the latter can be a problem with difficult to access starter motors tho.) if all wiring is ok its most likely the motor itself, if you can get to it give it a tap with something and see if it makes any difference. If it starts after a tap then its most likely the starter motor.

No, it makes no sense. You have V-8, right? Removing the exhaust manifolds will not provide any help in accessing the starter. The starter is in the middle of the engine.

I had the same problem with my 1992 LS400. My Lexus mechanic fixed it by replacing the MAF(Mass Air Flow).

This used to drive me crazy when the weather was hot.
