That clearly is a sensor problem you should take it up to auto zone get a read out then have them write down what sensor is bad
And most likely you get it on eBay or rock auto.com for 1/4 of the price auto zone is charging

I have the same van and the problem I had that was causing this was the fuel pump....

If you don't do much highway driving with your van you may have a carbon build up try some fuel njctor cleaner and mid grade gas for a tank or two If you go with the fuel additive rev your engine and watch for moisture and carbon from your exhaust contine to do this till exhaust is clear do not over rev ie do not hold it to the floor press he accelerator pedal and release

I have same problem, Chevy told me it was probably a seal affecting fuel air ratio and when warm seal expanded to stop air leak. I forget which seal but they also said to check plastic case around seal because it will warp and may also need to be replaced. It was located near top center of engine. Please let me know if you figure it out.
