did you make sure the ports to and from the egr valve are not blocked with carbon? And it is possible you have a bad new part. they dont make things like they used to.
On your model car you have to replace to EGR valve and clean the ERG port. To clean the port you have to remove intake maniflod. Use a drill bit and drill the build up out. Don't drill any metal, just the carbon build up. Honda make a kit for this repair, but you got to buy the EGR kit and the drill bit kit. Most people just replace the EGR valave and clean the port. See picture below on where EGR port is located(right side picture).
good luck
EGR stands for Exhaust Gas Recirculation, which basically sends some of
the exhaust back into the engine cylinder again. Your error means that
there isn't enough of the exhaust gas making it back to the intake
manifold.
The first thing I'd check is the tubes coming off of the EGR
valve to make sure they aren't clogged or broken. Then I'd check the
EGR valve itself and the throttle body. The problem is usually:
1.) Carbon buildup causing a clogging.
2.) Broken hoses.
3.) Broken Valve.
Check the EGR vacuum regulator and make sure you have battery voltage to the solenoid. If voltage is present then check the regulator resistance. To check resistance you need to remove the wiring harness and using an ohm meter make sure that it is within your vehicles specifications. You can also try testing the vehicles Differential Pressure Feedback Sensor.
P0401: Isufficient Flow Detected in EGR system.
-clean the EGR port with the carburetor cleaner.
-clean the EGR passage inside the EGR valve with the caburetor cleaner.
Reset the light, if the light still coming back, then replace the EGR valve.
Perform a valve clearance inspection and adjust if necessary. Tight or sticking valves will cause a misfire or rough running concern. Note: Perform valve adjustment when the engine is stone cold. 4cyl: INT 0.26mm EXH: 0.30mm (V6 engine are hydraulic and no adjust necessary.) Also check for a sticking or stuck open EGR valve. For diagnostic purposes only: Remove the EGR valve. Place a business card over the EGR passage and bolt the EGR valve back on. This plugs the EGR ports and if the misfire goes away the EGR valve is partially stuck open and is defective.
Check for low fuel pressure. Check for low compression. Check cam timing. Check for a weak ignition coil. Also don't over look the possibility of worn or broken engine mounts which cause excessive engine movement and engine vibration. -hope this helps
Sounds like a throttle body or an EGR problem (both airflow related). There's actually a warranty fix involving your car from Honda. It basically cleans out the intake manifold and drills a larger EGR port.

remove the egr .clean the carbon out the intake where egr valve sits . use a small tiny tiny screw driver or old coat hanger clean out the hole then vacuum up carbon.put back egr valve.
Code P1491 is EGR lift insufficient. It is usually accompanied by P0401. If you have a v6 Accord, there was a recall for the EGR port plugging. Check with your Honda dealer to see if your car is covered under that recall. If you have a 4 cyl, plan on replaceing the EGR. In addition, you need to clean the ports in the engine on the intake and exhaust sides of the EGR ports. To do this, spray some carb/choke//intake cleaner into the ports to saturate the black exhaust build-up. Later, jam a coat hanger or spare dipstick (or some long flexible metal rod) into the parts to break up the black deposits. When your rod passes through easily, spray more cleaner to pass it through. Reassemble and start the engine. It will run rough at first, so you need to give it gas pedal input to burn off the carb cleaner. You should be good to go.
