It should be behind the glove compartment, squeeze the sides of door and let it hang down, Look uo and to the right side and you'll see the mode door, just to the right side of the the door will be the actuator motor

Have you checked the engine belt, may have broken or came off. If so, now you need to find the issue that caused this. Pully bearing down or tensioner issue.
There are two of them, both should be replaced, one should be behind the glove box on the HVAC box and the other to the right of the steering column on the HVAC box.
