There are several things it could be, so start out with the easiest first. It could be bad gas, i'd buy some Sea Foam and put that in the gas when it is on about a quarter tank.

then if that doesn't work try changing your fuel filter.

if this doesn't fix it let me know

Rought or shaky idle

If you have a rough or shaky idle, the problem could be the air intake is dirty. You will need to get to the butterly of the air intake and clean it out. There is a cleaner that is sold for about $5. Use a screwdriver to help keep open the butterfly. Spray the cleaner and use a dry cloth to clean the inside of the intake. This will help to clean up alot of the gunk in and around the intake. When you first start it up it still may have a rough idle for about 20 to 30 second but it will go away. There may be even a little smoke of the exhaust but will clear up. You will have a great idle again. Great day.

Check your fuel pump which is located at the top of the fuel tank. Dismantle and check the "o" ring which may have disintigrated if you are using Gasahol

1995 GMC Safari Loses Power when accelerating hard, rough idle

Do you have a check engine light on? Is this a 4.3 with central port injection? If it's running real rich it might have a fuel leak inside the intake, either a fuel pressure regulator leaking or a feed line to the injector.

1985 f150 rough idle

An exhaust manifold leak would not case it to run rough. Check vacuum lines that go to the in take manifold. If you have a line that is open it will get too much air at idle and cause a misfire intermittently. You also need to check plug wires. How long have the plugs been in the heads. Check the rotor and cap also.
