No adjustments. check fluid, replace the master and slave cylinders. Could be the clutch also. Depends on driving habits.

1991 Toyota 4WD 5 spd manual: It grinds when I try to put it in 3rd gear.

no body told, the "pickup" land cruiser, or 4runner, what?
3 CAUSES (MORE IN DETAILS)
1, MC/slave bad, 2:clutch bad. ,3: synchro ring bad.
gear lube full and clean right, ??????
toss dice land on

toss again land on 2.4L engine
it has mc/slave.

ONE CAN SHIFT TO ANY GEAR YOU WANT PARKED, CLUTCH PEDAL HELD TO FLOOR. shift all gears parked foot to floor clutch

or only grinds on the fly

if yes, the 3 gear synchro worn out or its ring cracked.
reverse is the canary gear, no syncrho on it.
and love to grind, any time the clutch drags for any reasons
you do know what drags means?
the clutch pedal , down causes the PP to release the disk 100%
and not drag at all, (example a cracked disk)
the car has MC and slave
and the MC has reservoir, is the clutch fluid going down each weak or worse.
if fluid drops or seen dry the mc/slave is bad, a very happy thing that. as it is a simple job not 10 hours labor.

causes of drag so many , and did them all me. my hands
mc/slave , endless times. (never assume it is good ,prove it)
bad through out bearing collapsed.
bad disc cracked. or warped.
bad PP pressure plate, cracked or warped or springs bad.
flywheel pilot bearing seized or partally
transmission front main input shaft splines galled, and makes disc hub stick, and not slide smoothly.

When stopped gears sometime grind when shifting into 1st gear(2001 Ford Focus)

Then the clutch is not releasing properly & the continual
grinding will wear the gear teeth & you may also have worn
syncros, that will require you to pull the trans & go thru it, as well
as do a clutch replacement

Hard shifting manual transmission - warm/hot

check the clutch operation especially the m/cylinder for failure. The fluid and the master cyl. is probably not disengaging the clutch enough. the rears will not necessarily grate as the synchros are working overtime to spin the gears for alignment because the clutch shaft is still moving under power from the engine.

well it got stuck originally right??might be time for a rebuild the cable does need the right amount of tension to push and pull,the linkage may need proper adjusting as well

Difficulty shifting 2000 Ford Explorer 5 spd. manual transmission

have you checked to make sure your clutch fluid is topped up? low hydrolic fluid can sometimes cause this.
