if it is an automatic ( make and model omitted in question) there is every chance that it is transmission oil from a burst cooler in the radiator tank. The fix is a new radiator or and air to air cooler core fitted in front of the radiator. If the engine is a diesel or an engine that has a sump oil cooler then the oil core has burst allowing engine oil into the radiator vial the coolant system. It may be from a failing head gasket where the oil hole to the rocker gear is leaking across to a water jacket
water in the oil ,can be from head gasket failure ,cracked head but most probably from corrosion behind the water pump impellor. If the engine has an engine oil cooler then the cooler unit is suspect. Check any unit attached to the engine that uses coolant for cooling
If the water is in the transmission oil then it will be the heat exchanger in the radiator tank and will require a new radiator
If no external leak and the transmission fluid colour looks white, the problem belongs to the automatic gearbox oil cooler. The oil cooler has broke mixing the gearbox oil and coolant causing this milkshake. It's a small radiator core type oil / water heat exchanger. It is cracked inside and transmission oil is mixed with engine coolant. Because the transmission oil pressure is greater than engine coolant pressure, the transmission fluid level drops. INSTALL OIL COOLER
-clean the oil cooler contact surface on the oil cooled mounting
-install a new O-ring to the oil cooler.
-temporarily install the oil cooler with the nut
-apply a light coat of engine oi on the threads and under the head of the relief vale
-install the plate washer and union bolt. torque 78Nm (800kgfcm, 58 ft lbs.)
-tighten the nut,torque 8.8Nm (90kgfcm, 78 inch lbs.)
-connect the 2 water bypass hoses to the oil cooler.
I had the same problem with my T5 130. My mechanic replaced the head gasket to no avail,then we checked the oil filter,that was ok.then we went on to a web forum and found out that the AXD engine suffers from water pump failure.We took it off and it was corroded,the seal had failed and was pumping water into the block.My advice,CHECK WATER PUMP FIRST!
