you need to back flush the system. take the hoses of the rad line to the in side rad. and flush it out. at same time may aswell to the rest of the system get all the muck out of the system. needs to be done evey few years.
very difficult to advise without heariing the noise ,opel vectra (vauxhall) nice motors these ,if noise stops in 5th then it could be the gearbox like the layshaft needle bearings caused by low or no oil in the gearbox ,remember 5th gear doesnt use the layshaft its on the end of the mainshaft behind the cover next to the wheel arch
