Hi, this is a defective OD shift solenoid that is located inside the trans pan. you will have to drop this pan in order to extract it for replacement. this solenoid, if damaged, cant initiate the shift sequence required for the OD to engage during operational procedures.
Simply replacing this solenoid will fix this issue.
NOw, concerning the erratic shifts. this is isolated within the transmission control module. it seems to be malfunctioning. you will need to replace this module as well.

Have a look at the transmission mount. Also look at the universal joints in the drive shaft.
If the transmission mount is bad or mushy it will allow the metal part of the mount to touch the crossmember causing a vibration.
A dry u-joint will also cause a vibration. Even if the u-joint isn't worn out or has visible play. Being dry will cause this. Are the u-joints factory? Do they have grease fittings on them? Have they been replaced? How many miles on them?
If the u-joints don't have grease fittings then they are most likely factory u-joints. Hope this helps. Good luck.

leave nut etc... off and place wheel on spindle, screw lug nuts on about half way and shake tire in and out untill spindle comes loose make sure u have vehicle supported securely never trust a jack!!

Adjustment
The wheel bearings on these vehicles are not adjustable. If the bearings become loose or make noise, they must be replaced.
