When giving gas wont go pass 20mph and making loud vancuum noise. what could it be....please need some help
A major blockage in the exhaust system, a major vac leak around the intake, or a loss of fuel pressure come to mind.

From what you are describing it sounds like it must be a valve lifter sticking. Try putting a cup of transmission oil in your crankcase, or pick up some suitable additive from your local parts store. I wish i could hear it and give you a better idea. Maybe let them listen to it at the parts store, and they can determine if it's a lifter or not. Let me know how you make out.

I had the same problem (same truck). I replaced the axle u-joint solved the problem. I recomend replacing both side at the same time.

well there are alot of problems that can cause this irst things first... have the codes checked these will tell you alot. or atleast point you in the right direction. I you are not able to get the PCM scanned then you are looking at alot of testing, check the fuel pressure, make sure the fuel regulator is holding the pressure correctly. then make sure the injectors are pulsing correctly. then on to the throttle possition sensor you need to make sure its ready the sweep proporly, then onto the idle air control motor if its equiped with one. well that should keep you busy or a while if its none of those then ill keep working down the line.
