first thing to do would be a transmission service, the filter may be clogging up which reduces the pressure, if that does not work you may be looking at a new torque converter and or pump.
It sounds like your transmission is having a problem going into and out of overdrive. In drive if you are not accelerating hard through this range, the transmission should go into overdrive and you should see the engine RPMs drop. A bump can occur if the overdrive solenoid is misbehaving (for example, not releasing promptly when you depress the accelerator or engaging promptly when entering this range). You will not see a code unless the solenoid has failed. You should be able to verify this by watching the RPMs carefully when you feel the shudder.
Shuddering is a good sign of bad axle joints. You can look at the axle boots yourself, if they are ripped, they should be replaced. If you hear clunking or feel shuddering or wobbling, then axle will need to be replaced.
